Can anyone explain to me what is the difference between this two options. I tried the both and get the same results, I can’t see any difference at all.

Holding down Ctrl and hovering the mouse cursor over the button will give an explanation of what the button does. (in most cases)
You should be able to deduce the difference.
I believe the only difference between “Append” and “Insert” is where the subtool is placed on the list. Append always adds the subtool to the bottom of the list. Insert adds the new subtool directly below the active subtool.
I am not sure if its just me but I get some UI glitches when I use Insert. Some UI elements don’t update. So I usually just stick to Append.
