I have a model with 1.345 million polygons. When I ran Zremesher, it wipes out these small pieces. Is there a way to avoid that? Is this something that has to be manually retopologized?

Something to keep in mind about ZRemesher is that it does not fuse any geometry. If you feed it 2 separate meshes, you will get two separate meshes back.
It looks like what is happening is that those pieces are separate meshes from the main form. Since they are long and thin by the time ZRemsher reduces the polycount there’s not much left of them.
To improve your results, fuse the mesh together with Dynamesh or Live Boolean first prior to ZRemeshing. If you mark them as separate polygroups and use the ZRemehser “Keep groups” feature, you’ll get better topology with them. This will force Zremesher to draw loops aligned to that polygroup border, which should keep those slender features from being absorbed into the main body.
