Modeling a long sleeve shirt? [collar area]

Just curiouse to know what approach people would take to modeling something like a long sleeve shirt? Particularly around areas like the collar.

Would you start with a basic form and sculpt the collar in or would you model the colar and other details first? Same would go for any other clothing with collars like suit jackets etc.

I would probably do the collar and cuffs as extractions, I might do the shirt as one too come to that, depends how large it needs to be and how much movement I would want. but certainly the collar I would have a s a separate tool.
